Hospice is a unique medical, emotional, and spiritual support system for patients and families facing terminal or life threatening illnesses.

For immediate referral assistance, call 912-537-0063 (800-477-4758 toll-free). A Community Hospice Referrals professional will work with the patient/family to determine what level of service is needed.

Community Hospice accept any patient who is terminally ill and meets hospice qualification guidelines regardless of their ability to pay. Community Hospice does not bill the patient or their family for hospice service.

Caring for someone at the end of life can be physically and emotionally exhausting for caregivers. Hospice helps lift that burden and helps maintain, and often improve, quality of life issues that may develop.

Our multi-disiplinary healthcare team works with the patient and family to provide any support needed - inpatient or at-home - medical, social, emotional, and spritual.

Community Hospice services are available regardless of the ability to pay and without regard to race, creed, color, religion, sex, national origin, or handicap.
